高齢者のサイロトロピンと甲状腺ホルモンの意義



“年齢自体は甲状腺および甲状腺ホルモンへの影響は少ない。加齢にて、甲状腺は萎縮し、頸部の後ろに移動する。T3は若干低下するが、生体内の変化スピードは少し。甲状腺疾患は加齢とともに増加する。”

ってのが、いままでのお話でしたが、

甲状腺ホルモンや甲状腺刺激ホルモンの状態は、高齢者では、thyrotropinの増加と死亡率低下が関連。若干甲状腺ホルモンが高いほど死亡率増加。
やや低い方が良いのか?

【デザイン】前向き、観察、住民ベースの1912-1914年の2年間生まれの87%のLeiden 85-Plus研究。
569名の参加者で85-89歳までフォローアップ
【結果】
thyrotropinと遊離サイロキシン(fT4)は日常生活能力、うつ症状、認知障害と関連がない。

ベースラインのdisabilityや健康状態補正後、thyrotropin値増加と死亡率低下は関連がある。

SDあたりの死亡ハザード比は
・サイロトロピン2.71mIU/L増加毎0.77(95%CI 0.63-0.94)
・遊離サイロキシン0.21ng/dL(2.67 pmol/L)増加毎1.16(95%CI 1.04-1.30)

【結論】
超高齢者の一般住民では、thyrotropin高い群は特にadverse effectはないし、寿命増加によるものであろう。
しかし、高齢者に治療すべきでないというエビデンスは今後の課題で、よくデザインされたランダム・プラセボ対照臨床治験が必要。

内容より、すごい長いスパンの研究というほうを感心します。


日本では昔っからこういうpopulation-baseの研究を軽視していた感じがあります。久山町くらいでしょうか。確かに日本は戦後工業化により田舎から都市部への住民移動が会ったわけで、研究としてもなかなか難しかったのでしょう。

リンパ級数が少ないと老人では死亡率が高い。必ずしもトータルのリンパ球だけではなく、CD4+、CD8+、CD16といったサブセットも関連が有る。
