CKD患者で、透析前のうつ状態が、その後のアウトカムと関連

CKD患者で、透析前のうつ状態が、その後のアウトカムと関連する・・・という話は以前から”modification of immunologic and stress responses”、procytokineによるneurochemical な影響、栄養状態・食事・運動などのコンプライアンスなどと関連することなどが議論されていた。

MDD合併CKD患者も、早期介入により、果たして、CKDアウトカムが改善するか？

透析前CKD患者の２０％までうつ罹患と推定。Hedayatiらは、２６９名の透析前の２６７名の前向きコホートで現行の大うつ病エピソードと予後（透析開始、入院、死亡）不良との相関を検討。

major depressionの頻度２１％(3%-31%)で、大うつ病エピソードの存在は、合併症状況や腎障害重症度と独立してアウトカム不良のリスク増大
