ACSM/AHA Physical Activity and Public Health in Older Adults(高齢者対象)
 


2007年の健康成人(18-65歳)運動推奨
1. 良好な健康状態を素行心・維持するため、18-65歳の成人は身体活動性のあるライフスタイルを維持しなければならない I (A)
2. 中強度の好気的(持続:endurance)運動を最小30分週5日、もしくは、高強度( vigorous)好気的運動の最小20分週3日をしなければならない I (A)
3. 中強度・高強度運動の組み合わせはこの推奨に合わせ施行してよい。たとえば、週2回30分の早歩きとそれ以外の日は20分のジョギングなど IIa (B)
4. 中強度・高強度運動は日常生活の軽労作(例:セルフケア、皿洗い、デスクでの軽いツールを使った運動)や極短時間運動(例:ゴミ出し、ストアや駐車場までの歩行)に加え行う
5. 中強度好気的運動とは、早歩きや心拍増加を気づく程度運動で、最小30分まで1回運動を最小10分以上継続して積み重ねていくべき I (B)
6. 高強度運動はジョギングで代表されるが、呼吸数増加、心拍の増加をもたらす運動である7. 加えて、筋力と耐用性を維持・増加させるため、少なくとも週2回は身体の大きな筋肉の運動を行うべきである IIa (A)
8. 身体運動活動性と健康は用量依存的関係があるので、個々人の運行機能をさらに改善する希望があるなら、慢性疾患リスクや障害リスクを減少させる。不健康な体重増加を予防するには身体活動の最小推奨量以上に行うことでベネフィットをもたらす I (A)



medical newstoday(http://www.medicalnewstoday.com/articles/78619.php)では以前のガイドラインとの違いとして・・・
1.中強度身体運動の明確化:週5日を最低限として推奨
2.高強度運動(強化運動vigorous physical activity)を原則推奨に取り込んだ
3.明確化:中強度-、高強度運動活動性は、健康上のベネフィットがあり、運動の種類が推奨に合致する
週単位の量(強度×時間)をベースに組み合わせを行い、METsという単位で特異的な活動性の数値とする

4.明確化:好気的運動を、日々の日常生活身体運動を加えること
ルーチンの軽い強度の運動、すなわち、ウォーキングや食品の買い物や10分以内の駐車場まで歩くことやゴミ出しといった労作のような日常の軽い労作を推奨運動に加えることとなっている。現代の生活では身体運動が少ない場合、中等度強度の運動を少なくとも10分加えること。しかし、中強度・高強度身体運動(仕事のための早歩き、ショベルによるガーデニング、木工)を10分以上のひとかたまりとして行うこと場合は推奨の代用としてカウントしてよい

5.多いほどよい
運動推奨最小量は運動不足による慢性疾患リスクを減少させるが。1995年推奨のように用量反応関係は明確である。

6.短時間運動でもOK
目標30分の身体運動のの累積という概念をオリジナルの推奨では導入しているが、少々混乱もある。最小10分単位にすべきである

7.筋肉強化運動を今回から含む
今回はこの筋肉強化を推奨の核となっている。

8.擁護の明確化
“aerobic”、enduranceなどという言葉を、身体運動の種類などとともに明確にした


定義に関して頻回に用いられるようになっている

運動総量を、強度、期間、回数として表現することが一般的で
特に、中強度(moderate-intensity)、高強度(vigorous-intensity)を
中強度:3.0-6.0METs
高強度:6.0METs超
と、言明化し、今後運動強度の表現に一般使用するよう推奨している。






老人のガイドラインだが、基本的に同じで、7-9が特異的のようでそこだけ拙訳

Summary of physical activity recommendations for older adults – 2007.

1. To promote and maintain good health, older adults should maintain a
physically active lifestyle. I (A)
2. They should perform moderate-intensity aerobic (endurance) physical activity for a minimum of 30 min on five days each week or vigorous-intensity aerobic activity for a minimum of 20 min on three days each week. I (A) Moderate-intensity aerobic activity involves a moderate level of effort relative to an individual’s aerobic fitness. On a 10-point scale, where sitting is 0 and all-out effort is 10, moderate-intensity activity is a 5 or 6 and produces noticeable increases in heart rate and breathing. On the same scale, vigorous-intensity activity is a 7 or 8 and produces large increases in heart rate and breathing. For example, given the heterogeneity of fitness levels in older adults, for some older adults a moderate-intensity walk is a slow walk, and for others it is a brisk walk.
3. Combinations of moderate- and vigorous-intensity activity can be performed to meet this recommendation. IIa (B) These moderate- or vigorous intensity activities are in addition to the light intensity activities frequently performed during daily life (e.g., self care, washing dishes) or moderate-intensity activities lasting 10 min or less (e.g., taking out trash, walking to parking lot at store or office).
4. In addition, at least twice each week older adults should perform
muscle strengthening activities using the major muscles of the body that
maintain or increase muscular strength and endurance. IIa (A) It is
recommended that 8–10 exercises be performed on at least two
nonconsecutive days per week using the major muscle groups. To maximize strength development, a resistance (weight) should be used that allows 10–15 repetitions for each exercise. The level of effort for
muscle-strengthening activities should be moderate to high.
5. Because of the dose-response relationship between physical activity and health, older persons who wish to further improve their personal fitness, reduce their risk for chronic diseases and disabilities, or prevent
unhealthy weight gain will likely benefit by exceeding the minimum recommended amount of physical activity. I (A)
6. To maintain the flexibility necessary for regular physical activity and
daily life, older adults should perform activities that maintain or increase
flexibility on at least two days each week for at least 10 min each day.
IIb (B)
7. 転倒リスクを有する地域居住老人、転倒リスクを減少するため、バランスの維持・改善運動を行うべきである。 IIa (A)
8. 身体活動性に関係する医学的問題を一つ以上持っている老人では、治療の有効性安全性が状況を改善する方法で運動を行なうべき  IIa (A)
9. 老人は推奨される運動のタイプに帰着する十分な身体運動を獲得するプランを持つべきである IIa (C)  身体運動が治療である慢性疾患を有する老人では予防・治療を亢進する単一のプランを有すべき。 老人では、時間をかけた、身体運動活動性を増強する漸増(or 階段状)のアプローチがプランとして行われるべき。推奨レベル以下の活動が相当月数なされることが存在することは当然であり、その運動を漸増すべきで。老人は定期的に自己モニターを行い、その能力の改善、健康状態を再評価しながら行うべき。


個別化軽視の介護保険下の一律な運動処方ってのは、とても危険で、非科学的・・・・・・
