慢性HPとNSIPのCT鑑別

慢性HP(Chronic Hypersensitivity Pneumonitis)、NSIP（Nonspecific Interstitial Pneumonia ）、IPF（ Idiopathic Pulmonary Fibrosis）のCT鑑別の後顧的研究


Chronic Hypersensitivity Pneumonitis: Differentiation from Idiopathic Pulmonary Fibrosis and Nonspecific Interstitial Pneumonia by Using Thin-Section CT

【目的 】薄切CTの正確性後顧的評価
【Materials and Methods】
66 患者 (男性　36 , 女性　30 ; 平均年齢, 58.8 歳 ± 10.9 [SD])
診断判明例：chronic HP (n = 18), IPF (n = 23), or NSIP (n = 25)

【結果】 慢性HPのCTの最良の特徴は、" lobular areas with decreased attenuation and vascularity, centrilobular nodules, and absence of lower zone predominance of abnormalities＜線量吸収と血管構造の減少を示す小葉区域、小葉中心性の粒状影、下肺優位の病変がない＞ (P ≤ .008)”

NSIPのCT上の最良の特徴は、"relative subpleural sparing, absence of lobular areas with decreased attenuation, and lack of honeycombing＜胸膜直下に病変が少ないこと、線量吸収減少領域が小葉領域にないこと、蜂窩構造がないこと＞ (P ≤ .002) ”

IPFのCT上の最良の特徴は"basal predominance of honeycombing, absence of relative subpleural sparing, and absence centrilobular nodules ＜基底部主体の蜂窩、比較的胸膜直下sparingの欠如、小葉中心性粒状影の無いこと＞(P ≤ .004)”


信頼しうる診断は70/132(53%)で、70読影のうち66読影、94％が正確な診断であった。完全なコホートは80％で、診断に関する観察者間の一致率は良好であった。 (κ = 0.77–0.96)

【結論】慢性HP、IPF、NSIPのCT特徴は約５０％において正確

慢性HPをのぞけば、NSIPとUIPの鑑別は比較的容易？

NSIP患者の48/50読影（96％）で“ predominant pattern of ground-glass and/or reticular opacity with minimal to no honeycombing＜蜂窩肺を伴わないかほとんど無い、すりガラス状陰影 and/or 網状陰影主体のパターン＞”がみられる

一方、UIPの90％の予測値として、蜂窩肺の存在が主な特徴(P < 0.001)
UIPはNSIPに比較して、胸膜直下・patchyな病変が多い(P < 0.001)

NSIPとUIPのCT診断の正確性は、それぞれ73％、８８％
κ値は0.72

NSIP:

NSIPの組織学的特徴は“ spatially homogenous alveolar wall thickening caused by inflammation and/or fibrosis”で、空間的な、時間的な均質性がUIPと鑑別しうるNSIPの特徴である。

NSIPは線維化のパターンとして、肺胞壁が線維化と軽度間質性炎症によるびまん性の肥厚を示し、"fibroblastic foci"は存在しない。

過敏性肺臓炎は、このレビューではRB-ILDとの鑑別を問題にしており、喫煙歴により修飾される可能性が記載されている。
