脳内微小出血:抗血栓治療薬との関連

"that strictly lobar microbleeds are related to cerebral amyloid angiopathy, whereas microbleeds in a deep or infratentorial location result from hypertensive or atherosclerotic microangiopathy."(脳葉内出血はamyloid angiopathyと関連するが、深部・テント下病変は高血圧、動脈硬化性微小血管症と関連する)という仮説
や、 Rotterdam Scan Studyなどの脳葉内出血の話題が多くなってきた。

抗血栓治療薬未使用者に比較して、脳の微小出血が血小板凝集抑制剤使用者で多い(補正オッズ比 [OR], 1.71; 95% 信頼区間 [CI], 1.21-2.41)

抗凝固薬と微小出血の相関は有意ではない(OR, 1.49; 95% CI, 0.82-2.71)

厳格なlobar microbleedは、carbasalate calcium使用者(補正OR, 1.16; 95% CI, 0.66-2.02)よりアスピリン使用者(未使用者比較補正OR, 2.70; 95% CI, 1.45-5.04により多く見られる。この相違は良薬剤の同様量使用者比較でさらに広がる。


横断研究で、血小板凝集抑制剤は脳の微小出血と関連する。さらに、アスピリンと、carbasalate calcium使用はlobar microbleedに違った関連を示す。


先行して、脳内の微小出血が、全般認知機能障害をもたらすという報告があった。
