シンビコート近々発売予定・・・あらためて取り上げてみた。

それほど優れた薬剤なのだろうか・・・なんだか疑問に思えてきた

"formoterol/budesonide randomized"でpubmed検索すると、formoterol/budesonide AMDが fixed doseのsalmeterol/fluticasone propionateを凌駕する報告は認めがたい。

Salmeterol/fluticasone propionate (Seretide/Advair Diskus [dry powder inhaler] or Seretide/Advair inhalation aerosol [metered-dose inhaler])が、固定投与量なのに対して、シンビコートはAMD(Curr Med Res Opin. 2004;20(2):225-40.)、SMART（CHEST December 2006 vol. 130 no. 6 1733-1743）などの症状に応じた用量自家判断戦略を戦略としているように思える。

成人・青年群での、salmeterol/fluticasone propionate と formoterol/budesonide の固定用量比較はequivocal（Drugs. 2009;69(13):1799-828.）とされているが、一部に、急性増悪比較では前者の優越性が報告されている。

持続性喘息患者の以下の比較

salmeterol/fluticasone propionate combination (SFC) 50/250 microg one inhalation twice daily (bid)(要するに、日々 S 100/F 500）
vs
formoterol/budesonide combination (FBC) 6/200 microg two inhalations bid（要するに、1日　F 24/B 800)

急性悪化率は有意に時とともに両治療群とも減少したが、SFCはFBCに対する優越性が、維持治療群として中等症・重症急性増悪に対してあった。

stable-dose SAL/FPは、FOR/BUD　AMDに比較して、急性悪化減少、HRQOL改善に関して優れている。

LABA単独では抗炎症作用は弱い、あくまで、付録的効果。

although both budesonide/formoterol and salmeterol/fluticasone combinations are safe and effective, the pharmacological and pharmacodynamic differences between the LABA and ICS components of the two combinations allow maintenance and reliever therapy only with budesonide/formoterol. Adequate and controlled clinical studies indicate that the budesonide/formoterol maintenance and reliever regimen is the best current use of combination therapy, providing superior overall asthma control while avoiding over-reliance on SABAs or prolonged therapy with higher-dose ICS.

・・・と、前の記述と異なる報告がなされている。

プライマリ解析では、治療失敗率は、SALM/FP vs. BUD/FORM差異なし (RR 0.99, 95% CI: 0.83–1.16, p = 0.86; 3 RCTs）
セカンダリ解析で、hospitalisations/A&E visits増加(49% increased risk with SALM/FP vs. BUD/FORM, RR 1.49, 95% CI: 1.07–2.08, p = 0.02）・・・これは背後の吸入ステロイドの量が異なりフェアではない。

１１月下旬に、シムビコート発売記念講演に行ってきたが、前述の報告に全く触れず、最後の報告をまるでプライマリなメタアナリシスのごとく、洗脳していた。・・・シムビコート自体は非常によい薬で、rescue useに使用できるなど治療オプションの多い良い薬なのに・・・残念な講演会であった。
